Holiday Bible Club Permission Form

Our Holiday Bible Club runs this year from 1st – 5th August.

Please complete the permission slip as soon able possible to pre-register your child. Separate forms need to be completed for each individual child that attends. Forms can be emailed to legacurrybibleclub@gmail.com

Please note that emailed forms will need to be signed on the first day your child attends.
